If you are a new patient, as you would expect, there is paperwork to fill out before your treatment starts.
Once you book an appointment either online or by phone call, you will be invited to fill out onboarding forms prior to your visit. You will receive a confirmation email with a link to your onboarding forms
You are strongly recommended to complete these forms prior to your visit as accurately as possible.
